Clubfoot is a relatively common birth defect in which there is a deformity of the foot. There are many different treatment options for clubfoot. The two main options are surgery and the Ponseti Technique. The Ponseti method for clubfoot has only become popular in the last 10 yrs but was widely used in east Europe prior to that. The Ponseti approach uses stretching and mobilization to try and move the foot towards a more corrected position. After this a plaster cast is applied to hold it there. This process is repeated, usually weekly, over several months until the desired correction is achieved. The evidence of this Ponseti method is good which is why it is so widely used. Unfortunately, a few still need surgeries to achieve full correction, but not as many as in the past.

The blood circulation in the body is extremely important as it is how all the different nutrients that we take in via food and then processed by the stomach are actually moved to all the different parts of the body. The heart is responsible for moving the blood around. Obviously it is extremely important that the circulation be health or parts of the body will not be supplied with the nutrients that are needed, nor will waste products be removed from them which are another important function of the circulation system. There are many conditions that can affect the circulation systems. Heart disease and heart attacks are probably the most commonly known ailments. With any disease that affects the heart, and then there is going to be a problem with circulation as the heart will lose some of its ability to pump the blood.

One less well known ailment is a condition known as peripheral vascular disease (or poor circulation). This occurs when there is a problem with the circulation in the arms and legs. More often than not, the legs are much more commonly affected. The most common cause of peripheral vascular disease is atherosclerosis or a hardening of the arteries. There are many risk factors for this such as a lack of exercise, too much cholesterol in the diet, have other conditions such as diabetes and a poor family history of the condition. If the condition exists the legs are often cold due to the lack of blood flow, there will be no pulses felt and the skin often becomes fragile. The biggest problem with poor circulation in the lower limb is that any time a cut or sore develops, there may not enough nutrients available for the healing to take place and fight any infection from developing. This can have potentially very serious consequences for those with this condition.

The plantar fascia is the long ligament that attaches the heel to the forefoot, meaning that the plantar fascia plays a significant role in supporting the arch of the foot. The plantar fascia is extremely strong and needs to be as it is part of the foot’s natural arch support mechanism, known as the windlass mechanism. If this mechanism does not function properly or is overloaded then there are a number of things that can go wrong with the plantar fascia such as plantar fasciitis which is a very common musculoskeletal problem of the foot.

The Vibram barefoot running shoes have become the latest craze in the running community. The Vibram fivefingers were originally developed for use in yachting and have now been widely adopted by those who want to run in a shoe that is close to being barefoot while wearing a shoe. The Vibram five fingers is really just a covering for the foot and each toe with no cushioning and no support. As it is so minimal the Vibram five fingers mimics the action of barefoot running as the shoe does not alter the way that the foot functions like a traditional running shoe would. There is some controversy around the Vibram Five Fingers as it can a factor in getting an injury if time is not spent adapting to them after a life time of running in the traditional running shoe. There are several class actions against Vibrams at the moment!

As the toning shoes are unstable, the muscles have to work harder giving a training effect and a tone up. There is biomechanical evidence that toning shoes do this, but there are no clinical trials that show that toning shoes can do this. Other toning shoes make claims about being able to change the posture and help with low back problems. There are currently some legal issues for some of the companies involved in the toning shoes sector due to the health claims that they have made for their products that are not supported by the evidence.

As long as they are achieving all their developmental milestones and they can get the heel to the ground if they want to and there are no signs of any neurological problems, then it is probably better to ignore the toe walking as just a phase that they are going through. In other words, kids are shorter and they are toe walking as they want to be taller. Often it is just that simple. However, if there are behavioral issues, developmental issues, they cannot get the heel to the ground and there are some neurological signs present, then the toe walking should be fully assessed as there are a number of potentially serious problems that can cause toe walking.

A tarsal coalition is a bony bridge in the rearfoot. A tarsal coalition occurs when there is a failure during early development of some bones to separate into individual bones and they develop with a bridge of bone between them. Early on a tarsal coalition is not a problem as the bones are not full developed and are partially cartilage. As the bones more fully develop a tarsal coalition becomes more painful. The treatment for a tarsal coalition is usually to use foot supports to restrict the motion of the area so that it is not painful. Surgery is also used to remove the bone that is causing the problem. The most common tarsal coalitions are a talocalcaneal and the calcaneonavicular coalitions.

This is a big hint as to the best way to deal with smelly feet is to get rid of the excessive sweating. This is done by using insoles, socks and shoes to absorb the moisture. There are also come topical medications that can help this as well. Going barefoot is a good way to help remove the moisture. If these good foot hygiene measures do not help the smelly feet, then there are agents that can be used to mask the smelly foot.

Most of the pain of sinus tarsi syndrome is on the outside of the ankle and there is often a history of a previous ankle sprain. Sometimes there is no history of an ankle sprain in sinus tarsi syndrome and it can happen in a very flat or pronated foot. The most common treatment for sinus tarsi syndrome is manipulation.
